Services and Referrals
Our goal is to empower patients and their caregivers with the tools and knowledge needed to make informed, health-conscious decisions about their nutrition, fostering positive health outcomes from the comfort of home. Services are provided at no cost to the patient.
Referrals
Referrals are accepted from Health Clinics, Community Outreach organizations, Physician Offices, and Outpatient Dietitian offices.
After a referral is made, the Dietitian will contact the patient to make an appointment.
The number of visits is based on the need and will be confirmed after the initial visit and assessment.

Eligibility Requirements
To qualify for a no cost visit, patients must:
Have a medical diagnosis that can be managed or improved with diet changes.
Be motivated to change.
Be willing to allow someone in their home.
We offer more
The Ready Kitchen fills a vital gap in healthcare by offering in-home nutrition services to those in need, but who may not otherwise have access.
For those who do not meet the no-cost eligibility criteria, a fee-for-service is also available with a portion of the cost being donated back to meeting our mission.
